Forms and Waivers
These off-site travel forms contain event specific details and any mode of travel. The forms contain liability and medical release/consent. The forms may be kept by the group leader or with the person throughout the event. It is recommended to use these for each off-site event for minors. Young Adult ministries may want to modify an adult form for high-risk events, like sports or foreign travel.
For events with minors: These forms are an example on how a parish might communicate expected behavior and guidelines (and any consequences) to youth and parents for ministry events. These are samples only, and should be modified per event and parish. Youth Ministry leaders may build a Code of Conduct into the Travel Release Form to reduce the number of forms a parent needs to sign and request parents to discuss the guidelines with their minor child, rather than the child signing the form.
This form is recommended to be filled out annually for all youth (minors) participating in a routinely gathered ministry program. It contains information on youth and parents as well as a medical release for any on-site emergencies. It also contains a blanket Audio Video Release.
This form is recommended to be filled out annually by all volunteers who may be driving their own private vehicle to, from or for an off-site youth ministry event. It is especially important for those who will be transporting children, youth or vulnerable adults.
This form is helpful when a parish is hosting an event for minors and allows other parish groups to attend. The hosting parish may request other group leaders to complete this form to verify their adult leaders are Safe Environment cleared.
This form is recommended for documenting any incidents including, but not necessarily limited to, those which require disciplinary action such as vandalism or illegal activities, as well as any accidents or injuries which may occur.
This form is needed to document calls made to Child Protective Services. Volunteers or employees of any diocesan entity should have this form with them during the time of the call, whenever possible, and should present it to the entity Safety Officer once complete.
